Caring for Heart Health

There are many ways to lower the risk of heart disease. In fact, the World Heart Federation reports at least 80% of premature deaths from heart disease and stroke may be avoided. Take practical steps toward a healthier heart with preventive care, making lifestyle changes, and practicing self-care. Sutter Health Plus members have access to a variety of programs and services, many at no cost share, to help manage heart health.

Preventive Care
Preventive care helps protect against disease and provides early detection of certain health conditions, which can lead to better health outcomes. At yearly checkups, providers often suggest health screenings to help spot potentially serious conditions before they become life-threatening. It is important to know that it is safe to visit a provider for preventive services and not to delay care.

The annual health maintenance guidelines, Partnering for Your Best Health, helps members understand the latest immunizations, health, and screening recommendations.

Integrated Care Management
The Sutter Health Integrated Care Management (ICM) program is available for members who would benefit from disease management, including for heart failure. The program utilizes collaboration between members, their primary care providers, their healthcare team, and other care management programs and services. Led by specialized care managers, such as specially trained nurses, social workers, certified health coaches, and pharmacists, participants receive support to overcome barriers to care, including care transitions, complex care, healthy lifestyle coaching, and education for medication management.

Health Coaching
Need help achieving health goals? Sutter Health Plus members have access to a Health Coaching program at no cost share. The program provides one-on-one training with a personal health coach by telephone to uncover barriers to self-management, set goals, and make healthier lifestyle choices for maintaining a healthy weight, managing stress, and tobacco cessation.
