Whether it’s managing a healthy weight, improving mental wellness, or just living a healthier lifestyle, a variety of resources are available to support Sutter Health Plus members in achieving their wellness goals—many at no cost. Here are a few ways to help stay healthy new year.
Creating a Plan for Success
Take time to identify lifestyle changes that might have the most positive impacts on health. Then, select a few reasonable goals and write them down—this can significantly help in accomplishing them. Schedule time during the week to take simple steps toward each goal and track progress in a journal. It can also be helpful to list available tools, resources, and support networks.
Health Coaching Support
Reaching out for support can make a difference in engagement and motivation. Members have access to personalized one-on-one health coaching for maintaining a healthy weight, managing stress, and tobacco cessation at no cost. Coaches work with members to uncover barriers to self-management, set goals, and make healthier lifestyle choices. Appointments are by phone and the first call lasts no more than 20 minutes. Managing Health—anytime, anywhere
Through My Health Online, members can schedule in-person or virtual visits with a Sutter clinician, connect with their care team, renew prescriptions, view lab results, and access health information, 24/7. The free mobile app is available through the App Store and Google Play. Additionally, the online self-service Sutter Health Plus Member Portal provides convenient access to important forms, documents, ID cards, and information about benefits, copays, and claims for members whenever it’s needed.
Scheduling Routine and Preventive Care
Routine checkups and preventive care exams can help to detect health issues early when they may be the most treatable. Preventive care services, including but not limited to preventive care screenings, exams, and immunizations are covered at no cost share for members. Caring for Mental Wellness
Mental wellness is an important part of overall health. Members may self-refer to in-person or virtual visits with a participating behavioral health provider through U.S. Behavioral Health Plan, California for mental health and substance use disorder treatment services by calling 855-202-0984. Online resources are also available anytime at liveandworkwell.com, including crisis support phone numbers, a behavioral health provider locator tool, articles, self-assessments, and informational videos.
