Virtual Access to Primary Care

Sutter Health Plus members in the San Francisco Peninsula area now have access to TeraPractice, an innovative virtual-first primary care experience that offers easy, mobile access to personalized care.

This flexible care model comes at the same cost-share as a primary care physician (PCP) visit and includes access to the Sutter Health Plus network of specialists.

With TeraPractice, a dedicated care team—a PCP, a nurse and a health coach—offers a tailored and seamless experience through the use of video visits, phone consults, email messages and in-person visits when needed.

TeraPractice is ideal for busy individuals who want convenient primary care that fits their schedule, whether they’re at home, work, or on the go. People who have chronic conditions may find the expanded access and health coaching services especially helpful in monitoring their health. TeraPractice is a great option for those who seek a stronger relationship with their care team and want greater shared decision-making and flexibility in their health care.

Dr. Yumi DiAngi Taylor, a Sutter-affiliated Palo Alto Medical Foundation primary care physician (PCP) at TeraPractice, brings care directly to her patients through this virtual setting. While she practices in a virtual-first model, Dr. Taylor’s care team can also schedule in-person appointments as needed.

“We are building trusted relationships with our patients and getting to know them on a very personal level,” said Dr. Taylor. “Our virtual-first model means that we can provide longer or in-person appointments when needed, or quick ones when we can resolve an issue through a message or phone call.”

How TeraPractice Works
Members can expect the same services that they would receive from a traditional PCP with the additional benefits of flexible scheduling, personalized coaching, video visits and on-the-go messaging. This level of access can help reduce the need for in-person appointments, as members can reach out to the care team to ask questions and treat symptoms as they arise.

TeraPractice also participates in My Health Online, a secure online patient portal that allows members to pay bills, renew prescriptions, see test results, and more.
